Payment Policy
Medical services are provided and billed by David Grice, D.O.,P.A.. Our providers participate in most of the major health insurance plans in the region. Below are the major health plans which we currently accept. If you do not see your insurance plan listed below you can call the customer service phone number on your card to see if Dr. Grice is a provider for your plan. Please be aware that with any health plans, there may be deductibles and copayments which you are responsible for, and will be collected at the time of service. It is the responsibility of the patient to know their insurance benefits. Our staff will verify your insurance at the time of service, but there may be exclusions to your benefits.

Questions About Your Bill
What billing or insurance information will I receive?
Once your insurance has paid, you will receive an EOB (explanation of benefits). This will tell what was paid to the physician, what was applied towards a deductible if applicable, what adjustments were made, and what you may owe the provider.
How long will it take to get things settled with the insurance company?
Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.
Why am I getting bills from physicians?
Each physician who provides professional services will send you a separate bill from his or her office. These physicians will include:
- your own physician
- other physicians who conducted tests and procedures such as pathology reports if you had a specimen sent to a lab for diagnostic purposes. These charges are separate from Dr. Grice's charges and are billed separately. If you have questions regarding these charges you will need to contact the lab where the specimen was sent.
I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?
If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
